Update an insurance import

Replace the VaultFile on a pending or rejected InsuranceImport before it is submitted. Corrected extracted policy data is reserved for a later backing-service release and returns 422 until then.

Replace the uploaded Certificate of Insurance and re-run extraction. This consumes one import attempt, bounded by maxAttempts.

Extracted data is deliberately not writable. It is the extraction job's own output — there is no upstream write path for it, so a caller-supplied correction would have nowhere to land.
