Link a coverage to an insurance monitor

Links one of the payee's existing InsuranceCoverages to this monitor. Partial update via JSON Merge Patch (RFC 7396).

PAYEE-SEATED: linking is the payee asserting which of their policies satisfies the payer's requirement, so the payer receives 403 and a non-participant 404.

The coverage requirements are NOT editable here. They belong to the InsuranceMonitorConfiguration this monitor points at, which is immutable once monitors exist.

Link one of the payee's existing InsuranceCoverages to this monitor. The server validates ownership, expiry, accepted source type, policy type and every limit floor before advancing the monitor; a coverage that fails them lands the monitor NonCompliant rather than being rejected.
